Zimbra trigger amavis fitering by chance

Hi all:
 
Around 10% not all  email same domain or different domain will trsigger false postive . ANY reason why and slove?
Already set internal whole network by pass setting . It improve some  but still around 10% email will filtered and drop ...is the false postive set by the users their own?
