multiserver stup http proxy: 502 bad gateway nginx

I have just setup a new multiserver environment. I got everything working for the most part except for the web proxy. I have followed several guides in the wiki and the official 7.2 admin guide but i can not get consistent results from the setup. Sometimes is works other times (80 to 90%) i get the 502 bad gateway error. What could i be doing wrong.
This is what the ngnix log says:

2012/05/07 23:42:18 [error] 21077#0: *166 connect() failed (111: Connection refused) while connecting to upstream, client: 76.110.185.119, server: , request: "GET / HTTP/1.1", upstream: :80/", host: "mail.example.com"

please let me know if you want specific values from my config or zmprov. I usually dont post everything at first since it can be overwhelming on the first post
