Unable to Rename Account following upgrade to Zimbra 8 OSE

Hi,
I upgraded last night from Zimbra 7.2.1 OSE. Due to a broken pipe, I had to restart the install. I removed all references to the 8.0.0 upgrade attempt from /opt/zimbra/.install_history and restarted the upgrade. The second attempt at the upgrade appeared to complete as expected.
According to zmcontrol -v, I am indeed running Release 8.0.0_GA_5434.RHEL6_64_20120907144639 CentOS6_64 FOSS edition.
This morning, I was trying to change a user's email address (she wanted to switch to her new married name) and although I was able to change her "last name" I could not change her actual email address from first.oldlast@... to first.newlast@.... I got this error (I changed the user name for privacy reasons):
Server error encountered

Message: system failure: unable to rename account: first.oldlast@domainname.com Error code: service.FAILURE Method: [unknown] Details:soap:Receiver

How can I fix this? Thanks in advance for your help!

PS: I have been combing the forums for an answer to this, but have not found one as yet. If this has been answered elsewhere, please direct me to the relevant post (since I can't find it myself).

I have continued to read forum posts and simply cannot find anything related to this problem.
Should I try to run the installer "on top" of the current installation, or is there some database structure integrity check that I can run?

I also get this error when trying to update SOME users' passwords via the admin web interface:
Failed to save the changes to the account.

Message: system failure: unable to modify attrs: LDAP error: - unable to modify attributes: zimbraPasswordHistory: value #0 provided more than once Error code: service.FAILURE Method: [unknown] Details:soap:Receiver
